Primary and Community Health in South Western Sydney Local Heath District are looking for friendly, enthusiastic and passionate Social Workers to join our team across various roles.The Social Work Department offers a District wide support service to families with psychosocial vulnerabilities. We work alongside targeted home visiting nursing programs, support various paediatric clinics and strive to provide better outcomes for children in South Western Sydney.Working as a Social Worker in the South Western Sydney Community Health setting looks like:

The Social Work Team is responsible for providing responsive psychosocial interventions and support to families across SWSLHD. The Social Work Team supports families from a strengths-based model, providing holistic care within trauma informed, culturally safe interventions. Social Work ensures psychosocial supports and interventions are established in collaboration with the family and are adaptive to reflect their changing needs.The Social Work role is diverse as it works with various internal and external stakeholders to provide service collaboration, advocacy, case management and consultation. The roles require an understanding of diverse family structures and various Health Service models.We are looking for Social Workers who are passionate about making a difference in the lives of children & their families and who have experience in the areas of, mental health, domestic violence, disability and child protection.If you have experience in Child and Family focused intervention, case management and psychosocial assessments, we’d love to hear from you.Where You'll Be Working
SWSLHD operates its services on the lands of the Dharawal, Darug and Gundungurra lands and has a rich history of working alongside local Aboriginal communities.The Primary and Community Health Social Work Department are recruiting to a Talent Pool for future vacancies that may arise in the following teams:
  • Community Paediatrics Social Work Team – providing clinical support and psychosocial input to the Community Paediatrics team, supporting children undergoing diagnostic assessment and their families.
  • Targeted Home Visiting Child & Family Social Work Team – providing clinical support, case management, referral, advocacy and therapeutic intervention to families with children aged 0-5
  • Aboriginal Targeted Home Visiting Child & Family Team – providing culturally informed clinical support, case management, referral, advocacy and therapeutic intervention to families with children aged 0-5.
  • Family Connect and Support Social Work Team – providing clinical support and psychosocial input to SWSLHD families with children aged 0-17 for a period of 16 weeks.

  • Demonstrated experience in a health related field and ability to provide a range of Social Work interventions
  • Demonstrated high level communication skills (written and verbal) and computer skills
  • Evidence of commitment to clinical supervision and professional learning
  • Demonstrated understanding of, and commitment to quality processes, and the application of evidenced based practice within social work.
  • Must possess a valid Australian Drivers Licence (excluding learner driver's licences). If not already in possession, a NSW Drivers Licence must be obtained within 3 months of appointment.
