Step Thru Care is funded by the Commonwealth Government through the Western Victoria Primary Health Network (WVPHN) in partnership with Uniting Vic Tas. The program is designed to support clients with alcohol and other drugs (AOD) and Mental Health needs presenting both as a co-morbidity and independently. Clients are supported to move through stepped care interventions as determined by their current need with the option of remaining with the same practitioner as they move through steps.
This position is part of a multidisciplinary and collaborative care workforce providing structured psychological therapies to clients with complex AOD and Mental Health issues to improve their health outcomes. The service is provided at GCH sites and in the communities where clients live.
The Mental Health Practitioner provides person centred assessments and evidence based structured psychological therapies to enhance a person’s recover from chronic mental illness and co-morbidity of AOD. If appropriate, care coordination may be required in adjuncts to the therapeutic interventions and AOD supports for those requiring higher levels of care under a stepped care approach.
The service is provided via Telehealth and face-to-face at GCH sites and outreach into the communities of Grampians and Wimmera.
