 Position Summary

The Stroke Coordinator is responsible for the promotion and operation of Bass Coast Health’s Stroke Service. This includes the day-to-day coordination and care of stroke and Transient Ischaemic Attack (TIA) patients who present to Bass Coast Health. The role identifies and manages patients within the service throughout the cause of their presentation. 

The Stroke Coordinator:

  • Ensures the efficient and effective provision of patient care, based on clinical care standards and best practice principles within a collaborative multidisciplinary team
  • Participates in the coordination of person-centred, holistic care 
  • Supports excellence in clinical practice and promotes the development of the multidisciplinary team, within their scope of practice
  • Participates in local, service, and subregional quality activities
  • Demonstrates competence in the provision of person-centred care as specified by the registering authority’s licence to practise, education preparation and context of care
  • Demonstrate a sound level of skills in assessment, care planning, practice and evaluation in the provision of care to clients and their significant others.

Requirements of the position are:

  • Registered Nurse holding registration to practice with NMBA, or a relevant Allied Health Practitioner holding registration to practice with AHPRA
  • Relevant tertiary qualifications (Bachelor, Masters or equivalent recognised degree)
  • Experience working in acute, sub-acute, community, or rehabilitation settings
  • A current Victorian Driver’s licence
  • A current Police Check dated within 12 months of issue
  • Extensive clinical experience working with patients who have experienced a stroke
  • Demonstrated understanding of evidence-based stroke care
  • Working with Children Check
  • COVID-19 Vaccination in accordance with the Chief Health officer COVID-19 Mandatory vaccination Directions
  • Flu Vaccination
