The Supporting Accommodation for Vulnerable Victorians Initiative (SAVVI) Supporting Connections Practitioner is a position that is funded by the Department of Families, Fairness and Housing (DFFH), to assist Proprietors of Supported Residential Services (SRSs) in supporting their residents to live in a safe and supportive environment.
The Supporting Accommodation for Vulnerable Victorians Initiative (SAVVI) Supporting Connections Practitioner role fosters and maintains positive relationships with SRS Proprietors and staff, oversees the administration of funding agreements, contracts and financial functions of the SRS Proprietors on behalf of the Department of Families, Fairness and Housing. The role also includes assessing and addressing unmet health, social and behavioural needs of residents of Supported Residential Services (SRS) by working one-on-one with identified high needs clients and strengthening connections with local services.
To be considered for this position, applicants require:
- Relevant tertiary qualifications in welfare, social work, psychology, community development or other related discipline is desirable.
- Advanced knowledge and demonstrated experience in case management in an advanced administrative role.
- Experience working with clients with multiple and complex needs.
