About the Role:
The role of the Transition Services Clinical Nurse is to support and assist the Clinical Manager, Transition Services in the implementation and management of the Transition Services delivery model. The Clinical Nurse will be involved in the active search and triage for clients from acute care, determining eligibility of client, providing education to acute care staff, coordination of client services and continual monitoring of services provided by external and internal providers. The Clinical Nurse also involved in contributing to the reporting and quality monitoring mechanisms of the program.
About you:
- Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
- Registered Nurse/Midwife with at least 3 years, full time equivalent, post registration experience.
- Demonstrated competence in aged care nursing practice in accordance with the relevant standards
- Effective communication, problem solving, conflict resolution and negotiation skills.
