Community Care Manager

The Community Care Manager will oversee the management of the Councils Aged Care programs.  They will provide leadership and will ensure quality programs are delivered that is to a high standard, meets compliance and supports the client’s well-being and enhance their livability and lifestyle within community.
About the role

  • Full Time Position - 76 hours per fortnight
  • Katherine, NT
  • $111,273.93 – $119,833.47
  • 6 Weeks Annual Leave With 17.5% Leave Loading
  • 10 days Personal Leave
  • Remote Area Allowance
  • Relocation allowance of up to $5,000

Duties and Responsibilities

  1. Provide strategic advice and support to the Council, management, and staff.
  2. Develop and implement effective policy, strategic and operational plans, and direction in collaboration with the aged care team for the delivery of community services based on identified priorities and needs of regional community plans.
  3. Develop and implement all new processes, policies and procedures that reflect any legislative changes that affect the programs.
  4. Manage all aged care programs, Commonwealth Home Support Program (CHSP), Home Care packages (HCP), National Aboriginal Torres Strait Islander Flexible Aged Care (NATSIFAC) and the Indigenous Employment Initiative program (IEI) to ensure a high standard of service is delivered, contractual compliance is met, and programs are effectively utilised.
  5. Promote and develop the programs’ accessibility, range, and level of service delivery within approved budget parameters with the ability to seek further funding when required.
  6. Manage, lead, and develop high performing teams that are appropriately resourced with staff and systems for positive performance.
  7. Oversee and ensure all employees are correctly inducted and meet compliance requirements of the program.
  8. Ensure all emerging issues that may affect the programs are reported to the Director of Corporate and Community Services.
  9. Manage the budgets for each program ensuring a financially sustainable delivery model is in place.
  10. Continuously review budgets to ensure there is no overspends and all underspends are correctly applied for and utilised to benefit the programs.
  11. Actively participate in the budget development for each program and provide necessary feedback for budget reviews.
  12. Manage and coordinate all compliance and financial audits to be undertaken within the programs.
  13. Ensure the aged care team are provided with audit results and work with the team on any the development of community and quality improvement plans required and information is uploaded into LOGIC QMS.
  14. Oversee and ensure annual performance appraisals are conducted and training development plans are created for Community Care employees.
  15. Provide quarterly reports to the Director of Corporate and Community services for the purpose of Ordinary Council Meetings.
  16. Manage, prepare, and submit all reports in accordance with the Aged Care Programs.
  17. Manage centrepay and the Aged Care portal for consumers, Medicare payments in accordance with service delivery and the upload of DEX reporting ensuring all information is up to date, reflective of service delivery, accurate and submitted on time.
  18. Ensure Etools is up to date, complies with individual consumer requirements and budgets are reviewed to meet consumer individual requirements.
  19. Manage and ensure LOGIC QMS is being utilised to its full capacity and information is current and up to date.
  20. Liaise with the Grants manager to facilitate the development of external funding opportunities.
  21. Network and establish effective relationships with government agencies and other organisations to ensure VDRC is well placed to respond to opportunities as they arise.
  22. Oversee that all aged care assets are maintained, and improvements are undertaken as required.

 

Selection Criteria:

EDUCATION REQUIREMENTS

  • Tertiary qualifications in Public Health, Social planning, Management, or extensive experience (essential).
  • A Current First Aid Certificate (essential)
  • Current open NT drivers’ license (essential)

EXPERIENCE & KNOWLEDGE REQUIREMENTS

  1. Extensive experience in leadership roles in delivering high quality Aged Care services.
  2. Demonstrated ability to manage and support high quality service delivery including achieving and maintain registration and accreditation, strategic planning, financial management, business planning, staff mentoring, coaching and supervision.
  3. A demonstrated ability to meet strict compliance requirements of the Aged Care and Quality Safety Commission. Full understanding of the Aged Care Quality Standards.
  4. Demonstrated high level communication skills both written and oral including the capacity to prepare performance reports, operational and work plans, business cases, secure community investment and stakeholder engagement.
  5. Excellent communication and advocacy skills with the ability to effectively manage complex situations.
  6. Demonstrated ability to lead, manage and develop staff from a range of backgrounds and in a cultural context.
  7. Previous experience in working with Indigenous people or in situations requiring sensitivity to cultural differences.  Demonstrated ability to travel remote.
  8. Capacity to lead change management and to develop and implement organisation development programs and initiatives.
  9. Provide proved finance experience to develop and maintain program areas and meet strict compliance regulations in accordance with the Department.
  10. Demonstrated experience in effectively developing and maintaining strategic relationships with stakeholders, including all levels of local government, other levels of government and the community.
  11. Sound knowledge of the principles of Work Health & Safety
  12. Sound knowledge of etools data base program

Other Requirements
1. Criminal History Check (recent within 3 months)
2. A current Working with Children’s Card.
4. As this position is based in Katherine, you will be required to undertake extensive travel, overnight stays in remote communities within the Victoria Daly Regional Council area.
