We have an exciting opportunity for an Aboriginal Aged and Disability Support Worker to join our Aboriginal Health Team, Northern in supporting Aged and disabled consumers, their families and carers.
This role works in collaboration with other members of the Aboriginal Health team, community health services and external government/non government agencies to ensure the provision of quality support services to individuals and families under the Aged and Disability demographic.
As the Aboriginal Aged and Disability Support Worker, you will be work closely with other members of the Aboriginal primary health care team to provide social support, consumer directed care and advocacy services on behalf of the Aboriginal community including group settings. You will be responsible for assisting with the development and review of appropriate community programs and activities ensuring a multi-disciplinary and holistic approach to health care.
Eligibility
Applicants are encouraged to have knowledge of and experience in:
- Aboriginal culture, models of care and issues related to health service delivery, including domiciliary care and of community based services available locally and intrastate to assist with resolution of identified client issues;
- providing services to Aboriginal and Torres Strait Islander people including communities and groups, coordinating and delivering direct support services for the aged, disabled and those in need of social support, including carers
- working with or alongside Aboriginal people for service delivery and applying the principles of Primary Health Care; and
- using personal computers and the use of MS Office software
