The Aboriginal Wellbeing Coordinator will join our Care in the Community team and operate within our district wide Planned Care for Better Health program. This program identifies Aboriginal people over the age of 16 leaving a hospital setting (ED or Inpatient), who may require coordinated care within their communities to help support and manage their ongoing health, social and wellbeing requirements.You will operate within a culturally safe holistic model of care, assessing the needs of Aboriginal people in the community by linking them to services and support. As part of the Care in the Community team, you will be working closely with inpatient services, Aboriginal Health practitioners across the LHD, GPs, Aboriginal Medical Services, and the broader health & social care programs.The key focus is to ensure Aboriginal people are supported to access the most appropriate health and social care services, through assessed, planned and coordinated care in the community.This position will be working within the broader Care in the Community Service, Western Virtual Health within Service Delivery.To find out more about the role click to view theA career with Western NSW means you can challenge yourself, make an impact and shape the future of rural health.
