About the Opportunity
ACCP are currently looking for an amazing Aboriginal Health worker/Practitioner to join our team/family.
The ACCP team is a small but mighty multidisciplinary team made up of Aboriginal Health Workers/Practioners, Care coordinators, Dietitians, Admin, CNC’s, Clinical Team Leader, Facility Manager and ACCP Program Manager.
The team covers the whole SWSLHD with 3 office/clinic sites across the district – Wellama (Bankstown), Budyari (Miller) and ACCP offices (Miller).
The team has a big focus on not only the wellbeing of our clients but also each other.
The team work collaboratively in the provision of specialist services to Aboriginal patients and their families with chronic and/or complex needs in a culturally safe, patient centred way to facilitate optimal health outcomes.
The position aims to ‘Close the Gap’ in health disparities for Aboriginal people by addressing their chronic and complex health conditions and contributing factors.What You'll be Doing
The position is responsible for the provision of culturally safe, patient centred care coordination services to Aboriginal patients and their families with chronic and/or complex needs.
The position will work collaboratively with a clinician to provide assessment, care coordination, and education to patients, families and carers that takes into account clinical, psychosocial and cultural needs.Where You'll Be Working

  • Certificate III in Aboriginal & Torres Strait Islander Primary Health Care or relevant qualification OR willingness to complete and to undertake further training as identified.
  • Demonstrated understanding of the range of factors that have influenced the health status of Aboriginal communities.
  • Demonstrated experience delivering culturally safe health services to Aboriginal communities.
  • Ability to undertake health/social needs assessments that demonstrate a comprehensive understanding of Aboriginal culture and social issues in relation to health matters; and make referrals to community service providers.
  • Demonstrated experience working effectively as part of a multidisciplinary team to provide care coordination or case management services.
  • Demonstrated written and verbal communication skills and computer literacy.
  • Must possess a valid Australian Drivers Licence (excluding learner driver's licences). If not already in possession, a NSW Drivers Licence must be obtained within 3 months of appointment.
