CareU@Home is fast-growing home care service provider and NDIS service provider. We are seeking a dedicated Case Manager to join our team, providing exceptional case management and support coordination to our clients. As a Case Manager, you will play a crucial role in monitoring care services, referrals, and maintaining accurate case and budget records. This position can also be promoted to be a Team Leader depending on your experience and performance. Mandarin language fluency is essential with the ability to speak Cantonese also a bonus.
As always for us, attitude over experience. There are junior roles available for those early-into but passionate about the care industry. A track-record is required for more senior roles. Australian Sponsorship Visa option available for successful applicant (must have 2+ years experience).
Location: Full-time permanent position based in Croydon. Employees have the option to work from home.
Key Responsibilities
- Mange an allocated caseload, including the development, implementation and review of care plans and budgets
- Coordinate services and ensure efficient case management for clients.
- Conduct initial case assessments to identify client's goals and create customized care plans within individualized budgets, in consultation with clients, primary caregivers, families, and significant others.
- Regularly review and monitor care plans and budgets, making necessary adjustments based on clients' changing needs.
- Provide appropriate interventions based on clients' care requirements and preferences, such as arranging new services following health deterioration or hospital discharge.
- Match clients with suitable Care Workers to ensure quality service delivery.
- Collaborate with the accountant to supply accurate financial information for the preparation of clients' monthly statements.
- Undertake service coordination and liaise with the Aged Care Assessment Services, clients' GPs and other relevant service providers.
Requirements for Success:
- Proficiency in spoken and written English and Mandarin and/or Cantonese.
- Qualifications in social work/welfare, community services and/or allied health
- Strong conceptual and working knowledge of the community Aged Care sector.
- Demonstrated expertise in managing and developing community Aged Care programs.
- Excellent computer literacy skills.
- Possession of a current driver's license and access to a personal vehicle for work purposes.
- National police check.
