Position Title: Allied Health @ Home Coordinator
Employment Type: Ongoing part-time 64 hours per fortnight

Role description
The Allied Health @ Home Coordinator is a Senior Leadership position that will support the screening, intake and coordination of Allied Health care needs to @Home clients in the Hospital in The Home (HiTH), Geriatric Evaluation Management (GEM) @Home and Residential In Reach (RiR) programs. This position will work closely with the @Home Nursing Coordinators and leadership team to support the coordination and provision of expert Allied Health care to @Home Program clients.
This will include advocating for @Home client care providing health education, promoting health, assisting clients with goal achievement and working with other health care providers to ensure coordinated care for clients.
About you
Qualifications and Experience:
- Tertiary qualifications in relevant Allied Health profession
- Current AHPRA registration or equivalent professional association membership
- Drivers Licence
Essential:
- Minimum 7years clinical experience in relevant clinical area of expertise
- Demonstrated skills and capabilities required for discipline and program-based leadership in the context of @Home Care across acute, sub-acute and rehabilitation care phases.
- Strong communication skills, including the ability to effectively lead and maintain working relationships in a professional workplace and in clinical settings.
