We seek a Mobile Practitioner to join our mobile NDIS and Aged Care team across Sydney, Melbourne and Queensland. The role offers a lot of autonomy for the right candidate, allowing flexibility in working days/ calendar, a manageable and rewarding caseload and finally, an opportunity to progress to an area team leader role for the right candidate.
As a Mobile Practitioner, you will manage your caseload, conduct assessments, develop treatment plans, and deliver interventions.
Key Responsibilities:
· Conduct assessments, carry out therapy, & construct reports for our NDIS participants (Paediatrics and adults)
· Manage your caseload and maintain accurate participant records.
· Communicate effectively with participants, their families, and other healthcare professionals.
· Network with potential stakeholders to help grow the AHC brand in the local communities.
· Work collaboratively with other team members to ensure optimal client outcomes.
Requirements:
· Relevant qualifications and current registration with AHPRA
· Two (2) years’ of relevant work experience
· Experience working in the NDIS sector (Desirable but not necessary)
· Current Working with children checks.
· Current CPR
· Current driver's license and access to a reliable vehicle
· NDIS workers screening check (or willing to obtain)
· Excellent communication and interpersonal skills
· Ability to work independently and as part of a team
