A bit about you 

You are an engaging and empathic Outreach Worker with over 3 years’ experience. You hold an tertiary qualifications in relevant social work, community services, or another relevant qualification. You have experience working with people experiencing homelessness and marginalised populations experiencing psychiatric, medical and substance use conditions within a harm reduction model of care. You have a strong commitment to Social Model of health and trauma informed care principles. You love the thought of undertaking Assertive Outreach work with some of Melbourne’s most vulnerable people. Your passion for achieving health and wellbeing outcomes for people experiencing or at risk of homelessness is contagious.  

You know the importance of boundaries and frameworks but are an active listener and tailor your support based on the goals and aspirations to the person you are working with. You’ve got experience supporting a range of complex needs and bring a mix of best practice and responsive thinking to help us provide high impact support, without judgement!  

Key Requirements 
  • Recognised tertiary qualifications in relevant social work, community services or AOD qualification  
  • Police Check  
  • An understanding of the issues and implications of homelessness on clients with complex health and social needs 
A bit about the role  

You will be providing Assertive Outreach services as part of the Parallel Pathways Substance Misuse Team at The Living Room on Melbourne’s iconic Hosier Lane. It is a unique service combining health services, drop-in support, and social connectedness.  

The Assertive Outreach Worker will form part of a multi-disciplinary outreach team to provide high quality, compassionate, person-centred care and work in collaboration with clients and the multidisciplinary team to achieve identified goals and outcomes. The position will be critical to providing a health-focused outreach response which engages service users in order to create linkages with our Living Room Primary Health Service and a range of external health, outreach and housing services.  

Working within a social model of health, the position will form part of a multidisciplinary team approach to provide direct interventions and conduct referrals across a range of domains. The position will be required to work in the evening in an Assertive Outreach capacity 

This position is a part time role working shifts between 3 – 11pm Wednesday to Sunday  

This role is a 12 month fixed term position  

A bit about us

The Living Room is a primary health service, the team works with people aged 18+ to provide free healthcare and support to improve the physical, mental and social well-being of individuals who are, or at risk of experiencing homelessness. 

The only service of its kind in Melbourne, focuses on the social determinants of health to look beyond disease or illness, to see a person's overall situation. Our multi-disciplinary team works with individuals to identify goals and focus on harm minimisation, self-determination, and empowerment. 

We are a non-judgemental service with a strong focus on respect, client centred care and harm prevention. We work with clients on a personalised, tailored basis to address highly complex issues faced by people experiencing multiple and complex needs. 

Youth Projects is an independent, registered charity providing front line support to young people and individuals experiencing disadvantage, unemployment, homelessness, alcohol and other drug issues. We recognise the complex and varying needs of the people we work with and go above and beyond, 365 days a year, to provide pathways out of poverty.
