Sunbury and Cobaw Community Health is a not-for-profit community health organisation providing a comprehensive range of free and low-cost health, wellbeing and community services in early childhood, youth, families, adults, aged and disability.
Covering an area of over 1700 km2 from Sunbury to beyond the borders of Macedon Ranges Shire our 280 staff and 100 volunteers are based across four sites at Kyneton, Romsey, Sunbury and Woodend.

About the role:
Social Support Groups and Respite team is part of the Community Connections unit that sits within the Operations division. The Social Support Groups and Respite team have a vacant position for an Assessment Officer working with people who have been referred for a group program or 1:1 support to reduce social isolation and build connection with others The position is part-time (22.8 hours per week).
This position is responsible for providing initial Assessment and goal directed Care Planning for adult clients and Carers who are referred for Social Support and/or Respite Services at Sunbury and Cobaw Community Health. Clients live in the local community and may be referred via My Aged Care or may be participants of the National Disability Insurance Scheme (NDIS) or be holders of Home Care Packages. The role will require you to build rapport with clients and their carers, have excellent communication, planning, problem solving skills. Some travel across site may be required.

The successful candidate will be required to undertake and maintain a National Criminal History Check (NCHC), an NDIS Safety Screening check and hold a current valid Working With Children Check.
