About the role
Community Acute Post Acute Care (CAPAC) is a dynamic service that incorporates 4 essential programs; Hospital in the Home, Rehabilitation in the Home, Transitional Aged Care Program (TACP) and Healthy at Home. The service is a holistic multidisciplinary team that provides the right care for patients in the community to support them either recover at home or to avoid an unnecessary hospital admission.
Full and comprehensive orientation and training will be provided to the successful candidate on commencement.
Where you'll be working
As part of the nursing team you will have an array of roles, from assisting the client with their personal care, attending clinical observations, wound care, ensuring the client is able to attend their ADL management and assisting clients complete their home exercise program. You will work as a valued member of the multi-disciplinary team with the aim to increase and maximise our client’s level of independence.
TACP provides a multidisciplinary in home rehabilitation program to clients after an acute hospital admission. The team consists of nursing, physiotherapy, occupational therapy, podiatry, dietetics and social work combined with a full compliment of community support services.
About you
We are seeking an experience Enrolled Nurse that is able to work collaboratively and independently. You will also need to have skills that recognise a deteriorating client and escalate this appropriately. The successful candidate will be able to work within a large multidisciplinary team to provide high quality clinical care to clients in their homes.

All NSW Health workers are required to have completed a primary course of a COVID-19 vaccine which has been approved or recognised by the Therapeutics Goods Administration (TGA). Additionally, Category A workers are required to receive a booster dose three months after completing the primary course of COVID-19 vaccinations. Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
To be eligible for permanent employment in this position you must be an Australian Citizen, or a permanent resident of Australia, or a New Zealand citizen with a current New Zealand Passport. Employment of a temporary visa holder may only occur if no suitable local candidate is identified for the position; in this instance, you may only be offered employment in line with the conditions and expiry date of your visa.
