Job description

Multicultural Care (MC) is a not-for-profit, secular, community organisation, specialising in home and community-based support services for frail-aged people, people with disability, and their carers, from culturally and linguistically diverse (CALD) backgrounds. 

We provide a range of services and supports across Sydney’s Inner West, South West and South East, North Sydney including:

  • Home Care Packages
  • Short Term Restorative Care Packages
  • Commonwealth Home Support Program services
    • Respite (in-home, community, and centre based)
    • Domestic Assistance
    • Transport
    • Personal Care
    • Occupational Therapy 
  • Social support individual
  • Veterans Home Care (VHC) 
  • NDIS Supports
  • Care Finder Program
  • Community Visiting Scheme

The role will be responsible for:

  • Supporting and providing intensive assistance to help people in the care finder population to understand and assess aged care and connect with other relevant support in the community;
  • Specifically targeting people who have one or more reasons for requiring intensive support to interact with MyAgedCare, access aged service or other relevant support in the community.

Key Accountabilities

  1.  Care Finder Service and Support
  • Ensuring timely high quality care finder support that meet clients demands in accordance with care finder program guidance, framework, values and standards;
  • Developing and meet targets within the program work plan;
  • Continually review and refine care finder procedures and develop, implement and monitor quality improvement strategies and processes to support the growth of the business;
  • Providing input into policies and procedures relating to care finder services;
  • Work with clients in the community to understand their needs and help them access and connect with the supports they need. Support through registration, screening, and assessment
  • Leverage your networks and relationships with key agencies, other providers, and referral services to help identify, access and coordinate the supports your clients require. 
  • Utilise your extensive knowledge of the aged care sector, including the My Aged Care portal, to help your client navigate the system to obtain referrals and access to services
  • Follow up periodically with your clients to ensure they continue to receive the services they need and provide ongoing support where required

2. Reporting and Compliance

  • Maintain client files to meet MC and relevant compliance standards 
  • Report client and staff complaints, risks, hazards and feedback as per MC policies and procedures 
  • Provide reports to Engagement Program Coordinator and other nominated staff as identified 
  • Continuously communicate with consumers any changes or updates related to their service ensuring all documentation is accurately recorded 

3. Teamwork and communications

  •  Facilitating case discussion and other related activities to ensure continuous quality improvement of practice;
  • Resolving any workplace conflict in a professional manner and through the processes 

4. Values and Conduct

  • Positively and constructively represents Multicultural Care in all opportunities, role modelling the Code of Conduct and actively supporting the mission, vision and values 

 To be successful in this role you will have:

Mandatory:

Pre-employment:

  • Valid National Police Check
  • NDIS Worker Screening Check
  • Up to date COVID-19 Vaccination

Within two weeks after joining:

  • Comprehend Mandatory NDIS Orientation Module / NDIS Code of Conduct
  • Attend MC’s Online or One on One staff training on Abuse and Neglect and provide the participation certificate
  • Comprehend MC’s Consumer / participant’s journey

Essential

  • Fluency in Hindi, Vietnamese, or Arabic language.
  • Tertiary qualifications in a health-related field and/or relevant experience.
  • Experience in aged care.
  • Excellent organisational skills and the ability to prioritise work effectively.
  • Sound level written and verbal communication skills.
  • Experience with common computing applications.
  • A strong willingness to acquire the skills and knowledge to enable contributions towards the development and implementation of Care Finder program and initiatives to meet the needs of the business.
