An exciting and rewarding opportunity is available for a suitably qualified Health Care Worker – Community Support to join the Community Aged Care Services team at Portland District Health.
In this role you will assist clients with:
- Light, general housekeeping such as sweeping, vacuuming, mopping, dusting, wiping, bed making, laundry, and limited shopping
- Nutrition, assisting with meal preparation, medication support and personal care
- Provide in home respite or participate in social activities like shopping, outings and walks that assist them to stay active and connected with their community: including one on one and/or group activities
- Encouraging consumers to actively participate where possible to maintain their independence
- Support the individual’s choice to remain at home and participate in their community
About you:
- Certificate III or Certificate IV in Aged Care, from an Australian registered training organisation or recognised equivalent qualification.
- Demonstrated ability to practice collaboratively and work effectively as part of multidisciplinary HEALTH CARE team.
- Demonstrated understanding of the needs, issues, and sensitivities of people from diverse backgrounds and cultures.
- Excellent communication skills and ability to focus on delivering friendly, courteous, and respectful service to patients and residents.
