Job description

Part time fixed term – minimum 12 hours per fortnight

Available to 30th June 2024

An exciting and rewarding opportunity is available for a suitably qualified Health Care Worker – Community Support to join the Community Aged Care Services team at Portland District Health.

In this role you will assist clients with:

  • Light, general housekeeping such as sweeping, vacuuming, mopping, dusting, wiping, bed making, laundry, and limited shopping
  • Nutrition, assisting with meal preparation, medication support and personal care
  • Provide in home respite or participate in social activities like shopping, outings and walks that assist them to stay active and connected with their community: including one on one and/or group activities
  • Encouraging consumers to actively participate where possible to maintain their independence
  • Support the individual’s choice to remain at home and participate in their community

About you:

  • Certificate III or Certificate IV in Aged Care, from an Australian registered training organisation or recognised equivalent qualification.
  • Demonstrated ability to practice collaboratively and work effectively as part of multidisciplinary HEALTH CARE team.
  • Demonstrated understanding of the needs, issues, and sensitivities of people from diverse backgrounds and cultures.
  • Excellent communication skills and ability to focus on delivering friendly, courteous, and respectful service to patients and residents.

Portland District Health (PDH) provides an integrated health service for the community. The hospital was established in 1856 on the current site and now provides a comprehensive range of services that include:

  • Hospital based Medical and Surgical services, Medical Imaging (Radiology), Pharmacy and Pathology
  • A 24/7 Urgent Care Centre for emergency treatment
  • Residential aged care for 30 residents at Harbourside Lodge
  • An extensive range of primary and community services for the local community

PDH delivers services from a single campus, employs around 470 staff with an FTE of approximately 290, and has an active volunteer workforce. The People and Culture Unit sits within the Corporate Services Directorate, with other Units including Finance, Building Services, Food Services, Environmental Services, Executive Support, Administration and Health Information.
