This role will play a key role in optimising HCP client numbers and funds utilisation for mecwacare’s HCP Program. The role will also be responsible for promoting and raising the profile of mecwacare’s HCP Program to the communities and potential stakeholders; promptly responding and answering potential client enquiries in a consistent way; attending first home visit with new clients; facilitating and coordinating sign-up for HCP with individual clients; and providing information about available services, explaining the costs associated with HCP and completing initial setup of services.
In addition, this position will be responsible for delivering presentations and networking with community groups.
What you bring:
- Tertiary qualifications in Social or Allied Health, Health Science, or Registered/Enrolled Nurses with current registration.
- Previous experience in the Aged Care Sector.
- Previous experience with effectively engaging with a diverse range of internal and external stakeholders.
- Advanced customer service skills (written and verbal) and strong negotiation skills.
- Well-developed prioritisation and organisational skills with the ability to meet deadlines.
- High level proficiency with the Microsoft Office suite including Word, Excel and Outlook.
- Excellent presentation skills especially to community groups.
- Current Victorian Driver’s License to allow the use of a company vehicle for work purposes.
- Previous experience in case management
- Thorough understanding of the fees and charges applicable to Home Care Packages
- Knowledge and understanding of Aged Care funding and legislation as it applies to Home Care Packages.
- Knowledge of My Aged Care service provider portal.
