We are looking for a self-motivated, experienced person who enjoys a diverse workload with lots of variety.
The responsibilities of a Care Advisor are varied, and each day brings new challenges. As a member of a larger team, you will oversee a portfolio of clients, ensuring that you provide the necessary support for them to safely stay at home while working towards their goals.
To be successful, you will require;
- Appropriate qualification compliant with Australian Qualifications Framework (e.g. Associate/Diploma of Case Management and/or Community Services /Nursing/ or related field, or 3 or 4-year undergraduate degree in a health related field).
- Two years’ or evidence of substantial experience with case management, client assessment and care planning, in particular managing a caseload of clients from goal directed care principles and brokerage model of service delivery.
- Current Victorian Drivers Licence.
