We are supporting our client - a non-profit community aged care provider who has been providing quality in-home services in South East Queensland for 25 years. They provider of HCP, CHSP, DVA Community Nursing, ACVVS and Transition Care services.
About the Role:
The Home Care Package Coordinator, is responsible for the coordination, care planning and monitoring of HCP clients ensuring a quality service is provided and helping them remain living independently in their own homes.
For this role, you must have a minimum of 2 years Home Care Package Coordination experience.
You will have a manageable case load of Level 1 and 2 HCP clients and will enjoy building good relationships with clients, their families and other health professionals. You will be well-supported as you work closely with our other Coordinators, Registered Nurses and Allied Health teams.
This is a permanent full time office based role in Brisbane CBD with company vehicles available for client visits.

To be successful in this role you will have:
- Minimum 2 years demonstrated experience as a HCP Coordinator
- Demonstrated Home Care package knowledge including legislative requirements
- Strong organisational skills including the ability to problem solve, prioritise and multi-task
- Excellent communication and interpersonal skills with the ability to work well with others in a small team environment
- Strong computer skills including MS Office and care databases
- Current open Australian Drivers Licence
- Current First Aid and CPR
You will also need to be a permanent Australian resident and have a National Police Check (or willingness to obtain).
Ideally you will fully vaccinated - COVID19 and Influenza - though this isn't mandatory.
