About MulunguThe position is located within the Mulungu Aboriginal Corporation Primary Health Care Service and forms part of the Health Services team, which is responsible for providing services to support the corporate and operational responsibilities of the organisation. In meeting these responsibilities, a range of client support services are provided.
About the roleThis role is located in the Health Services department of Mulungu and is responsible for the coordination of care for patients with chronic and complex health conditions and is responsible for the maintenance and facilitation of improvement systems to support our clients.
Integrated Team Care Coordination involves purposely organising client care activities and sharing information among health service providers involved. This planned and managed approach ensures that the best health outcomes are achieved for clients and that health services are effectively and efficiently used.
About you- Possession of a diploma in Health Services and/or Nursing with a minimum of 3 years' experience in a similar role.
- Exceptional customer service skills
- Identify as Aboriginal and or Torres Strait Islander
- Advanced knowledge and skills in office management systems,
- Previous medical reception experience desirable
- Previous experience collating statistical data and report writing is desirable.
SC 1 Possession of a diploma in Health Services and/or Nursing with a minimum of 3 years' experience.
SC 2 Experience working with Aboriginal and / or Torres Strait Islander communities and demonstrated knowledge of Aboriginal and Torres Strait Islander cultures.
SC 3 Demonstrated advanced knowledge and experience in the Microsoft Office Suite of programs and basic knowledge of Pracsoft and Medical Director software with the ability to quickly acquire knowledge and work with new Information Technology systems and programs.
SC 4 Well-developed interpersonal skills with the ability to communicate effectively with people, particularly Aboriginal and Torres Strait Islander peoples.
SC 5 Demonstrated ability to provide leadership, training and co-ordination of systems and processes.
SC 6 Knowledge of and/or involvement in quality improvement processes.
SC 7 Knowledge of, or ability to acquire knowledge of human resource management issues including Workplace Health and Safety, Equal Employment Opportunity and Antidiscrimination.
Additional Factors- Possession of a current ‘C’ Class Driver’s Licence (Queensland)
- Willingness to undergo an Australian Federal Police Check
- Must hold or be able to obtain a Working with Children Blue Card
