We currently have an exciting opportunity for a Full-timeCare Coordinator Team Leaderto join our award-winning, trusted, family-owned business! Celebrating our 12th birthday this year, we are specialists in in-home care, transition care, respite care, chronic illness support, palliative care, and dementia support and consider all our clients and their families as part of our Angel family.
Based at our Head Office in one of Milton's oldest terrace houses on the Brisbane River, we are looking for a driven professional with care coordination experience, who is a great communicator, and enjoys leading a close knit, focused, and dynamic team.
THE OPPORTUNITY
As our Care Coordinator Team Leader, you will:
- Work with pride and feel fulfilled in coordinating your clients' care needs
- Onboard new clients
- Manage their care plans as needs change; personalising their support based on their individual goals to ensure the quality service we are known for
- Support a small team Care Coordinators, including the completion of end of month reports
ABOUT YOU
To be successful in the role you will have:
- Experience as a Care Coordinator (minimum 12 months preferred)
- Aged care industry experience with strong Home Care Package and NDIS knowledge (preferred)
- Strong decision making and problem-solving skills
- Demonstrated ability to manage own workload, multi-task and prioritise effectively
- Experienced in MS Office suite including outlook, word, excel and client database systems
- Ability to lead a small, cohesive team
- Willingness to take after hours phone on a rotating roster (shared with 7+ people) including evenings and weekends
