Grampians Health is seeking a dynamic, energetic clinician to work in the Transition Care Program and Aged Care Assessment Service. We have a role as an Assessment Clinician in Stawell available on a fixed term basis of 0.2 EFT and a Care Coordinator - TCP, Permanent Part Time – 0.4 EFT available.
You will be a Registered Nurse or have an Allied Health qualification in any of the following disciplines Occupational Therapist, Physiotherapist, Psychologist, Social Worker.
The position is a unique opportunity to work across the programs in community and healthcare settings to provide quality client centred care.
The Grampians Aged Care Assessment Service operates within the Local Government areas in the Grampians region. The Assessment Clinician will play an important role, as a member of a multi-disciplinary service, to provide a comprehensive assessment service primarily for aged members of the community and their carers. Transition care provides care coordination and short-term management and support, working with clients to meet restorative goals and transition from their health service journey.
Working with healthcare teams you will facilitate and foster best practise and service delivery to clients and their families.
Key Responsibilities
Skills and experience
- A post graduate qualification or working towards in relevant area (highly desirable) e.g. Community Services/practice, Gerontology, Aged Care, Chronic Disease, Cognition, Mental Health etc.
- Proven skills in assessment, management, care planning, goal setting and discharge planning for people with complex medical, aged and/or psychosocial needs.
- Proven experience in Care Coordination or Case Management
- Previous experience working in a multi-disciplinary team

This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2020 - 2024 or Allied Health Equivalent. The classification is Clinical Nurse Consultant Grade 3 (ZF4, ZG7, ZJ4, ZJ7) which ranges from $54.64 – 57.51 per hour.
All appointments are subject to a satisfactory police records check, immunisation clearance and Working with Children check and/or NDIS Worker Screening Check.
All employees must be willing and able to wear Personal Protective Equipment (PPE) when required, which at a minimum is a surgical mask. Additional PPE requirements are role-dependent.
