Care Coordinators use a solutions based approach to facilitate delivery of a range of Community Nursing services that support independent living in a person’s home. Care Coordinators coordinate and manage intake, assessment and review process, ensuring these meet consumers’ changing needs, state and federal funding guidelines and EGHS care policy and procedures.
What we OfferAbout the Role
The role involves coordinating a range of services and programs that enables clients to live independently and to identify and address shortfalls in service delivery with a focus on chronic and complex care. The Care Coordinator will use their clinical expertise to mentor and support clinical staff in the delivery of care.
Care Coordinator shift times include Monday- Friday AM & PM and Saturday-Sunday AM as rostered.
About You- Current registration with AHPRA as a Registered Nurse
- Demonstrated ability to assess, plan and coordinate care for clients with complex medical, psychological and chronic illness
- Knowledge of community nursing funding programs
- A post graduate qualification in management or interest in post graduate study will be considered favourably.
Appointment is subject to immunisation clearance (Including COVID-19) and current and satisfactory Employment Checks (Police and Working With Children Checks)
