We have an incredible permanent, full time opportunity for a Care Manager to join our team!
The Opportunity
Our Care Manager plays a pivotal role in supporting our Facility Managers to coordinate the delivery of healthcare services to patients. They work collaboratively with interdisciplinary teams, including physicians, nurses, social workers, and other healthcare professionals, to ensure the provision of high-quality care that meets the needs of patients. The Care Manager is responsible for assessing patient needs, developing care plans, coordinating care transitions, and providing education and support to patients and their families.
As a Care Manager, you will provide supervision and monitor Registered Nurses, Enrolled Nurses and Care Workers and exercise initiative in regard to clinical care matters to consumers in accordance with:
- The Aged Care Quality Standard
- The Code of Conduct for Aged Care
- The Charter of Residents Rights
- Organisational Policies and Procedures
The successful applicant will need to demonstrate sound clinical skills, managerial skills, be consumer-focused, have a solid understanding of aged care accreditation processes and an appreciation of current and future aged care funding models.
Duties & Responsibilities
- Coordinate and oversee in consultation with the Facility Manager, the Continuous Improvement, Occupational Health and Safety, and Training/Orientation programs, to ensure the overall delivery of care meets the Residential Aged Care Standards
- Effectively manage incidents, hazards and near misses and ensure impact is minimised with appropriate actions taken to improve systems, work practices and the working environment
- Consultation with medical practitioners, allied health and other health professionals to ensure positive outcomes for care recipients
- Provide recommendations and coordinate referrals, following clinical assessments, to specialised health professionals where necessary
- Ensure significant issues and changes in residents’ health is professionally conveyed to the Facility Manager in a timely manner
- Monitoring and identifying AN-ACC reviews, including ensuring that documentation is proficient, to ensure consumer care funding is optimised
What you’ll need:
- Current unrestricted RN AHPRA registration
- Bachelor of Nursing or equivalent
- Senior Clinical Nursing experience of 2+ years
- Knowledge of Aged Care Quality Standards (or the ability to acquire this)
- Satisfactory national police check within the last three years
- Tasmanian Working with Vulnerable People registration with NDIS endorsement
- Valid Australian Working Rights
