We are passionate about providing high-quality care and support services to vulnerable members of our community, and we are currently seeking a motivated Care Manager to join our team in the aged care and NDIS industry.
We are looking for a Care Manager who will be instrumental in fostering strong relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goals. You will create consumer-directed care plans, respond to client feedback, and effectively manage client issues and outcomes.

About The Position:
As a Care Manager, your responsibilities will include:
- Conduct thorough assessments of consumers' care needs and develop personalized care plans.
- Collaborate with consumers, their families, and healthcare professionals to establish and update care goals.
- Establish strong relationships with consumers and their families, serving as a primary point of contact and addressing any concerns or issues promptly.
- Educate consumers and families on available resources, support services, and care options to promote their well-being and independence.
- Ensure compliance with all relevant regulations, policies, and procedures, including maintaining accurate and up-to-date client records.
- Monitor and address any potential risks or safety concerns, implementing appropriate measures to mitigate them.
- Foster effective communication and collaboration among care providers, clients, families, and other healthcare professionals involved in the care process.
- Act as a liaison between clients and the organization, conveying feedback, concerns, and suggestions to improve service delivery.
Requirements:
- In order to establish your career within this position, you will require the following:
- Tertiary qualification in Health or a related field and relevant experience in care management evidenced
- Relevant professional experience in home care, care management, or a related healthcare setting
- Strong knowledge of home care regulations, industry standards, and best practices.
- The right to work unrestricted in Australia (evidence will be required)
- A current National Police Records check, dated no later than 3 months from application (or willing to apply for one)
- Working with Children Check (or willing to apply for one)
- NDIS Workers Screening Check (or willing to apply for one)
- Minimum 2 doses of the COVID-19 vaccination
- A current First Aid and CPR Training (or willing to obtain)
