About the Role:
Under a time-limited Commonwealth initiative to reduce pressure on ACT Emergency Departments (EDs) and public hospitals, an innovative model is being trialled. The Primary Care Pilot is aimed at strengthening partnerships between Primary Care, community-based care and the public health system to support access and care for patients.
The Pilot is a collaboration between the ACT Health Directorate, Capital Health Network, General Practitioners (GPs) and Canberra Health Services (CHS). The CHS Primary Care Pilot operationally reports to the Integrated Care Program under the Office of the Chief Operating Officer. The Care Navigation Team will assist in the delivery of the Pilot, providing liaison, navigation, care coordination and complex case management in partnership with participating GPs, selected non-GP specialist groups and community-based Allied Health teams.
This temporary position will provide critical clinical and project support, and leadership for a team of Registered Nurses and Health Professionals providing services to consumers referred to the Primary Care Pilot, their families, and their care supports. The Pilot aims to reduce pressure on ACT Emergency Departments and hospitals through innovative care pathways supporting and enhancing Primary Care capacity and improving the ability for consumers to remain well in the community. This position will work closely with internal and external contributors and stakeholders including the Pilot-dedicated Liaison and Navigation Team, Community Allied Health, Non-GP Medical Specialities, General Practitioners in Primary Practice, Capital Health Network, and the ACT Health Directorate project support team.
This position will also act as a central point for the support of monitoring and coordination for the Pilot, including data reporting, issues management, stakeholder liaison, and Pilot evaluation.

North Canberra Hospital - As of 3 July 2023 Calvary Public Hospital Bruce transitioned to CHS and became North Canberra Hospital. The transition will deliver a health system networked under one provider and will provide increased workforce opportunities for staff at both CHS sites. The ACT Government will be building a new northside hospital on the existing hospital campus in Bruce to meet the growing health care needs of our community, with construction to commence mid-decade providing more beds, increased services, and increased career opportunities.
