About the role:
This position will see you working in the Western Region of Uniting AgeWell's network based out of our Kingsville office. You will be the key contact for clients and play an active role in supporting our Care workers maintaining day-to-day service delivery and coordination of client services including: scheduling, customer service, records management, monthly reporting and document control.
- Develop and maintain relationships with clients and/or their carers to ensure care needs are being provided
- Service plan development and ongoing reviews
- Initial engagement with perspective clients
- Annual review of existing client services
- Monitor and manage group email inboxes, incoming calls and walk in clients and community members
- Ensure that client finance information is managed in a timely and accurate manner
- Ensure that the administrative requirements of the program are maintained in accordance with Departmental and organisational requirements

- Experience and knowledge in Commonwealth Supported Home Care Packages
- Previous administrative and rostering/scheduling experience in community services
- Ability to manage multiple requests in a fast paced environment
- Experience within the aged care sector
- Thrive in a team and also appreciate the one to one interactions
