1 x Care Planner position with Post Hospital Support Program

ABOUT THE ROLES

Carers ACT is in search of multiple Care Planners to assist carers or patients in maintaining their wellbeing. Throughout this process, you will collaborate closely with patients and/or carers to ensure appropriate support are in place, remaining committed to addressing their individual needs. 

For the Post Hospital Support Program, this is achieved by:

  • Engaging in hospital discharge and bed management meetings with participants, their care teams, and families or carers.
  • Conducting participant intake, perform needs assessments, and identify discharge goals.
  • Developing discharge care plans, outlining necessary non-clinical services for safe transition home or to a designated community setting, such as domestic assistance, personal care, meals, transport, and bridging supports.
  • Coordinating and arrange services from CATS and Carers ACT approved providers for up to 6 weeks, including accessible transport from hospital to home or community setting.
  • Monitoring Participant progress and adjust services to meet changing needs.
  • Facilitating connections to longer-term assistance, including other CATS Service providers or community care providers and supporting access to NDIS or My Aged Care services.

For the Carer Gateway Team this is done through:

  • Assessments with carers and/or care recipients that come through the Carer Gateway to develop Carer Support and Action plans that incorporate SMART goals and long-term change. 
  • Outreach and networking to promote the Carer Gateway and Carers ACT supports. 
  • Working collaboratively with internal and external services ensure comprehensive care arrangements are in place for carers and/or care recipients with care needs.
  • Maintaining transparent relationships with all carers and stakeholders by fostering relationships built on trust. 
  • Maintaining relationships with service providers to remain up to date with service offerings, capacity and referral pathways ensuring flexible brokerage can be delivered to support carers. 
  • Engaging with complex families in a supportive way. 

ABOUT THE YOU

We're looking for compassionate and empathetic individuals to fill these roles. The ideal candidates will have strong communication skills to engage and collaborate with care teams, participants, families and carer. They are detail orientated and be skilled in conducting needs assessments and developing care plans. Your qualifications and skills include:

  • Certificate IV in Community Services or Mental Health, Case Management or equivalent and experience working in the health and/or community care sectors. 
  • Previous experience with case planning and goal setting with a positive outcome for multiple people simultaneously. 
  • Demonstrated empathetic and flexible approach in managing the sensitivities of participant needs.
  • Excellent communication, presentation, and interpersonal skills.
  • Demonstrated ability to manage time effectively and prioritise workload. 
  • Strong commitment to teamwork and good practice. 
  • Ability to work with minimal supervision / direction. 
  • Intermediate to Advanced computer skills (Microsoft office & databases).

Additionally, you meet the following essential requirements:

  • Current Working with Vulnerable People Registration (with NDIS activities if issued after July 2021) or willingness to obtain.
  • Up-to-date COVID vaccinations
  • Current Police check 
  • Current Driver’s Licence (essential to undertake client meetings at external locations).
