Your Impact in the Role
Your efforts will support our strategic initiatives, enhancing the quality of life for carers and the people they care for across the ACT. This role is crucial, not just for its immediate tasks but for the lasting impact you will create in our community.
The key responsibilities and qualities for the Care Planner role include:
- Assessments: Conduct assessments and develop Carer Support and Action plans that incorporate SMART goals and long-term change.
- Outreach and Promotion: Engage in outreach and networking to promote the Carer Gateway and Carers ACT supports.
- Collaboration and Care Arrangements: Work collaboratively with internal and external services to ensure comprehensive care arrangements are in place for carers and/or care recipients with care needs.
- Transparent Relationships: Maintain transparent relationships with all carers and stakeholders by fostering relationships built on trust.
- Service Provider Relationships: Ensure flexible brokerage can be delivered to support carers.
Who You Are
You are an individual who has:
- Passion for Change: You have a strong passion for creating positive change in the lives of carers.
- Demonstrated empathetic and flexible approach: You have consistently shown empathy and adaptability in addressing the unique needs and sensitivities of participants. Whether it’s understanding diverse perspectives or adjusting communication styles, your approach is commendable.
- Excellent communication, presentation, and interpersonal skills: You excel in conveying ideas clearly, whether through written or verbal communication. Your presentations are engaging, and you foster positive relationships with colleagues and stakeholders.
- Demonstrated ability to manage time effectively and prioritise workload: Time management is one of your strengths. You efficiently allocate resources, meet deadlines, and handle multiple tasks without compromising quality.
- Strong commitment to teamwork and good practice: You actively contribute to team dynamics, collaborate effectively, and uphold professional standards. Your commitment to shared goals ensures a cohesive work environment.
- Ability to work with minimal supervision/direction: You are self-driven and capable of making independent decisions. You take initiative, seek solutions, and thrive even when guidance is limited.
Your Background will include:
- Certificate IV in Community Services or Mental Health or equivalent
- Experience working in the health and/or community care sectors
- Previous experience with case planning and goal setting
- Current Working with Vulnerable People Registration (with NDIS activities if issued after July 2021) or willingness to obtain
- Up-to-date COVID vaccinations
- Current Police check
- Current Driver’s Licence (essential for client meetings at external locations)
