About the Opportunity:
The Case Manager will join at an exciting time as we launch our new Lighthouse model at Woolooware Shores. Under this new model, we will provide the option for higher care services (including Home Care Packages) in our retirement living villages to help residents remain in the comfort of their homes for longer. This is an exciting new pilot project that if successful, will be run across our 25 retirement villages.

About the Role:
The Case Manager will help deliver an excellent customer experience to prospective residents, supporting them to maximise their care plan to meet their individual needs.
Other key responsibilities will include (but not limited to):
Ensure a smooth transition from sales intake to the start of care, establishing trust and credibility with prospective residents.
Coordinate Aged Care Assessments as care needs are identified.
Ensure the professional delivery of care and support services are provided.
Ensure all documentation is completed and uploaded to the respective system in a timely manner.
About You:
We are looking for an enthusiastic and committed individual to join this team and pilot project. The ideal candidate will have a mix of the following attributes:
Diploma of Service Coordinator / Community Services, or an equivalent qualification.
Previous experience within the Aged Care sector, with experience undertaking comprehensive assessments in a home care setting.
Attention to detail and exceptional customer service skills.
The ability to handle competing priorities and make quick decisions.
A current driver’s licence is also required.
