AddressOur client is seeking a Full-Time Case Manager for a position located in Dongara. As a Case Manager you will be required to case manage an allocated caseload of participants providing integrated case management. This will include actively engaging participants with the program, supporting participants to find sustainable employment and improving participants capacity to work by stabilising non-vocational barriers and building vocational skills and experience.
MAIN DUTIES/RESPONSIBILITIES
• Work in a Continuous Quality Improvement cycle to ensure case management practices are aligned with contractual obligations.
• Be a positive and motivational mentor encouraging participant engagement with the Program.
• Undertake assessments and goal setting to strengthen a participant’s ability to find and sustain employment.
• Provide individual coaching and support to a diverse range of Participants, which may include career planning, resume writing, job search support, improving employability skills, addressing personal issues, as well as improving skills, training, and education.
• Facilitate access and referral to internal MEEDAC projects and services as well as external specialist organisations.
• Secure sustainable employment opportunities through Participant referral to internal and external job vacancies as well as advocate to the MEEDAC Employment Support Unit (ESU) on a participant’s behalf to find work experience or employment.
• Assist with Post Placement Support and Employer Engagement when required.
• Document all Participant contacts in a timely and accurate manner and undertake all required administrative duties utilising internal MEEDAC designed tools.
• Servicing and maintaining Centrelink and or DOT clients if applicable.
• Assist with relief case management and activity delivery when required.
• Outreach servicing if required including managing travel calendar and outreach office locations.
• Mentor and peer training when required.
• Participate in staff meetings and staff development training.
• Travel for training and events.
• Other duties as directed by Line Manager.
REQUIREMENTS
• Valid Australian Driver's License.
• Australian Citizen.
• Appointment will be subject to a satisfactory Australian Police Clearance and other checks as required.
• Where a program or third party mandates a medical vaccination for service delivery or attendance requirements, this role may be subject to such a vaccination requirement as a condition of employment.
• Be able to adhere to MEEDAC Drug and alcohol Policy.
• The ability to maintain a sound level of understanding and adhere to MEEDAC policies and procedures.
• Be competent in the use of Microsoft Office applications.
• Have proficient written and oral communication skills, including the ability to communicate with people from a broad range of backgrounds.
• Have excellent time management skills with the ability to set priorities and to achieve outcomes in a demanding environment.
• The ability to develop and maintain positive, collaborative working relationships with peers, participants, and relevant stakeholders.
• The ability to work autonomously with limited supervision, exercise judgement, set priorities and schedule work to meet deadlines.
SKILLS AND EXPERIENCE
• Knowledge and experience of case management.
• Ability of work alongside participants demonstrating complex needs, including culturally and linguistically diversity
• Management and its practise with clients demonstrating complex needs and who may be culturally and linguistically diverse.
• Experience in working within an outcome-based environment to achieve positive results.
