Your Opportunity
The SPIN 180 program in Darwin provides comprehensive support to young individuals who are facing the risk of homelessness or have become disconnected from school, education, employment, or their familial and community networks. With a focused aim on intervention and prevention, our program endeavours to steer these vulnerable youths away from the perilous paths of homelessness and anti-social behaviour. We recognize the profound and far-reaching consequences such behaviours can have on their integration and functioning within the broader community.
As the Case Manager, you'll play a pivotal role in supporting young people and their families, offering referrals to external services, and guiding them towards positive changes in their lives. Providing person-centered case management to young people who are at risk of disengaging from education and family relationships, potentially leading to homelessness you will aim to empower these individuals, enhance their social inclusion, and create pathways to education, employment, and stronger family ties. Your key responsibilities will be to:

  • Receive and respond to all referrals promptly.
  • Undertake assessments to determine eligibility and induct young people into the program.
  • Develop comprehensive, positive case management plans with young people, using SMART goals and referring them to other support services as needed.
  • Engage young people into training/educational programs, handling paperwork and providing necessary information.
  • Help young people find and reach goals to leave the program, including connecting with education and training providers.
  • Work with the team to assess risks.
  • Lead case conferences with key stakeholders to minimize overlap and achieve the best outcomes for each young person.
  • Assist the Program Manager and other staff with complex cases or emergencies.
  • Organize, facilitate, and advertise group activities and special projects as needed.
  • Build strong relationships with young people and staff to improve service effectiveness and outcomes.
  • Collaborate with partner organizations to plan and support program participation.
  • Promote the program to young people and the community.
  • Keep individual files for young people according to Mission Australia's policies and agreements.
  • Prepare internal and external reports on participant numbers, activities, and results.
Requirements for Success
  • Degree in Community Services or equivalent qualification, with minimum one year of experience.
  • Proven experience in providing case management for young people facing complex issues.
  • Understanding of the challenges affecting young people at risk of homelessness.
  • Familiarity with Aboriginal cultural systems and protocols.
  • Previous work in a child-safe model of practice.
  • Proficient written and verbal communication skills.
  • Capacity to maintain confidentiality and professional boundaries.
  • Empathetic approach with a focus on strengths-based solutions and collaborative problem-solving.
  • Ability to develop and maintain accurate records.
  • Team player with the ability to work within a multi-disciplinary team.
  • Exceptional interpersonal skills for building relationships.
  • Proficiency in computer use, including keyboard skills and report writing.
