The Role:
- Work within a close and dynamic team to coordinate the delivery of our high standard of care to clients. Our intention is to meet and exceed client expectations and meet business requirements.
- Onboard new Home Care Package clients, as well as conducting regular care plan reviews in accordance with the clients changing needs.
- Communicate with staff and stakeholders regarding enquiries, referrals and follow ups via phone and email.
- Relationship building with clients and their families.
- Schedule services for new home care package clients: working closely with the Scheduling Team to assign services.
- Monitor Clients budgets.
- Enable our clients to lead fulfilling, independent lives through value focused relationships and quality support services.

The ideal candidate will have the following:
- Qualifications in Community Services, Nursing or equivalent
- Understanding of the Aged Care Industry and experience with Home Care Packages.
- Outgoing and approachable, with a strong focus on achieving client trust and satisfaction.
- Ability to interpret and apply policies and procedures.
- Highly developed organisational, interpersonal & time management skills.
- Efficiency across multiple IT platforms
- Excellent verbal and written communication skills.
- Focus on and skill in managing complex relationships.
- Experience and empathy in dealing with people from a wide variety of backgrounds.
- The ability to effectively work and communicate with multidisciplinary teams to achieve client goals.
- Ambition to be part of a visionary future for aged care.
- Beerburrum to Gympie and surrounds.
Please note that the successful candidate will have the following:
- First-Aid Training Certificate less than three years old
- CPR training certificate less than twelve months old
- Confirmation of your accepted NDIS worker screening status
- Confirmation of your accepted Working with Children screening status
- Proof of Covid-19 Vaccination - minimum 2 vaccinations
- Valid Queensland Driver Licence
- Birth Certificate, Passport, or proof of your eligibility to work within Australia.
