We are seeking a proactive and highly organised Case Manager.

Key Responsibilities
• Oversee client administration, CDC paperwork, including general administration
• Records management, including continual quality management and improvement
• Professional liaison with clients and carers
• Coordination of community-based needs
You will be supported by a rostering team and will be working with management in our fast-paced environment, supporting and driving our sales processes, as well as liaising with clients and our partners and stakeholders. You will be a key part of our small team in a growing organisation.
To successfully fill this role, you will have:
• Previous experience in Case Management with CDC and/or NDIS and Home Care Packages
• A reliable vehicle and current unrestricted Driver’s Licence
• Demonstrated proficiency in Microsoft Office Suites and Computer based systems
• Excellent verbal and written communication skills
