About the role
In this critical role you will draw upon your multifaceted problem-solving and analytical skills to provide home care plans and packages to clients and their families. Your strong understanding of care principles will help to tailor home care plans to individual client needs, while your warm interpersonal skills will help to ensure the high level of quality service that VMCH provides.
About you
You are a vibrant personality who enjoys working autonomously and has strong communication skills. We’re always adapting and improving, so you’ll be flexible in your approach and are comfortable in juggling multiple priorities at once.
In addition to your love to go for a drive, you will also possess the following:
- Tertiary qualification in Health or a related field and relevant experience in case management evidenced.
- Experience in assessment and development of care plans.
- Experience in setting and management of consumer budgets.
- Experience in provision of case management to consumers with complex needs.
- An understanding of Aged Care Reforms and implications for the organisation and consumers.
