About the role
We are actively looking for motivated individuals to join our committed Community Options team in the role of Case Manager. This part-time position is permanent and ready for an immediate start, with workdays falling between Monday and Friday. The main responsibility of this role is to evaluate and implement Case Management Packages for individuals facing the risk of premature or unsuitable admission to residential care, or those whose quality of life is negatively impacted and may have complex care requirements. Flexibility in terms of days and hours is offered for this position, with the possibility of negotiating school hours and remote work on certain days.
Skills and experienceKey Selection Criteria include:
Essential:
- Relevant qualification or experience in case management, welfare, social work, nursing or allied health
- Advanced communication and interpersonal skills
- Well-developed computer and documentation / reporting skills and willingness to learn required IT programs
- Demonstrated commitment to quality service to consumers
- Current Victorian Drivers Licence
Desirable:
- Demonstrated understanding of and skills in case management
- Recent experience with community-based services for people and their carer’s
- Demonstrated skills with advocacy, negotiation and conflict resolution
- Demonstrated ability to work effectively within a multidisciplinary team
