About the role
We are seeking a dedicated person for our Case Manager - Intensive Case Management Initiative position! This role sits within our Homelessness Case Management (HCMS) team and is responsible for providing a range of services to disadvantaged adults.
Responsibilities include
- Establishing effective relations with clients that encourages self-determination through persistent and assertive engagement techniques
- Providing brief intervention, service coordination, Case Management, referral, and advocacy to successfully achieve client goals
- Enduring accurate, professional, and timely maintenance of client records and data collection in accordance with relevant policies and procedures
- Building connections with internal TSA Programs, services, and streams; build and use interagency and community connections and partnerships
This is a Permanent Part-Time (30.4 hours per week) position, based in Flagstaff, West Melbourne. Salary and conditions are in accordance with the Social, Community, Home Care and Disability Services Award Level 5.
What you will have
- Diploma or Degree qualification in Social Work, Social Science, Psychology, or related field
- Relevant experience in the Social Services sector, including Case Management
- Ablility to demonstrate knowledge of homelessness, including context, causes, models, and approaches relating to best practice
- Experience working with internal and external stakeholders to both maintain and create new connections in the sector
- A current Australian Driver's Licence
