Key Responsibilities:
1. Person centred assessment and care planning
- In consultation with clients, carers and families, you will facilitate care assessment, care planning and review when needed
- Develop and monitor individual plans and budgets with regular reviews/case conferencing
- Manage any identified risks within principles of Positive Approach to Challenging Behaviours and Duty of Care, Dignity of Risk
- Development and maintenance of all documentation in client management systems
- Work within legislated NDIS and CDC frameworks
- Be aware of and comply with legislation and organisational requirements relating to Workplace Health and Safety
- Contribute to quality processes such as management of client feedback
- Actively participate in professional development activities (internally and externally) to maintain contemporary knowledge and skills as well as acquire new knowledge and skills
2. Client and stakeholder engagement
- Make relevant referrals, broker services and deal with suppliers as per care plan
- Develop and maintain relationships with community organisations to ensure a holistic case management approach
- Develop and maintain relationships with brokerage agencies to ensure quality services are delivered by external providers.
- Ongoing communication with clients, carers/families to ensure they are aware of rights and responsibilities to make informed choices.
3. Business Development
- Ongoing relationship management with relevant referral sources
- Develop links in the community to source new referrals
- Retention of existing consumers
- Participate in objectives and performance review to reach KPI’s set in relation to new and existing consumers.
You will be supported by a senior Case Manager, supporting and driving meaningful Aged Care and disability service delivery, as well as liaising with clients and our partners and stakeholders. You will be a key part of our small team in a growing organisation.
To successfully fill this role, you will have:
- Minimum 1year experience as Case Manager or bachelor / diploma in community or health or nursing background.
