The mission of Cystic Fibrosis Community Care is to increase the well-being and quality of life of people living with cystic fibrosis and promote broader awareness of cystic fibrosis. It does this through the provision of advocacy, support and information services, research, public awareness programs and fundraising.
ABOUT THE OPPORTUNITY
Case Manager - Vic Location - Part Time (30 Hours Per Week) - with hybrid working arrangements, with up to two days face to face required in office or at Clinic.
The Case Manager will assess, review and support cases referred to CFCC, and provide a range of counselling, practical support and social work services to adults and families living with cystic fibrosis.
Tertiary qualifications in counselling, psychotherapy, psychology, social work, community work or similar, and professionally registered membership with PACFA, ACA (level 2 and above) AASW, ANZACATA or equivalent, is required.
REQUIREMENTS
- Undergo probity checks, including criminal record checks and working with children checks as outlined in their Contract of Employment.
- Must be able to drive, provide, and maintain a valid Australian Drivers’ License.
- Vaccination compliance under the Public Health (COVID-19 Vaccination of Health Care Workers) Order 2021
KEY SELECTION CRITERIA
- Experience in counselling or a social worker role and the ability to demonstrate a professional work history. Health experience is an advantage.
- Ability to communicate in a positive and effective manner with clients, hospital clinicians, members, and supporters of CFCC, volunteers, colleagues, and representatives of other organisations.
- Proficient skills in the use of Microsoft 365.
- Ability to maintain up to date and accurate client files and program records, write professional reports, and provide relevant documentation to other stakeholders.
- Experience working in multi-disciplinary teams or with external stakeholders advantageous.
- Willingness and ability to comply with the policies of the organisation, with privacy, confidentiality, health and safety policies, and working with children.
- Willingness and ability to perform the range of occupational tasks required of the position including lifting light loads, driving, access via steps to buildings, self-care in dealing with the emotional demands of the role (including counselling in grief and loss).
- Ability to exemplify health-promoting practices to people with cystic fibrosis including responsible self-management of stress and fitness, and to reflect a healthy approach to respiratory wellbeing.
- Effective personal organisational skills and the ability to perform in an autonomous role.
- Experience or knowledge of the challenges faced by people with cystic fibrosis or other terminal chronic disease, or invisible illness is an advantage.
- Ability to work autonomously and utilise clinical reasoning for management of client support.
As well as:
- Tertiary qualifications in counselling, psychotherapy, psychology, social work, community work or similar.
- Professionally registered membership with PACFA, ACA (level 2 and above) AASW, ANZACATA or equivalent
