About the role -
We are seeking to recruit an experienced Case Manager/Care Coordinator who can communicate well with people of all backgrounds and approach every situation with care and compassion.
Key responsibilities include:
Development and coordination of Individual Support Plans, rosters, budgets and other mandatory documentation in line with the NDIS:
- Building and maintaining strong relationships with the participants, family members and stakeholders to achieve their NDIS goals.
- Maintaining safe and healthy workplace by implementing and adhering to Workplace Health and Safety Procedures.
- Completing NDIS documentation such as Participant Care Plans and Risk Assessments etc.
- Face to face meetings and regular face-to-face contact with our Participants and team of Support Workers.
- Maintaining accurate records as per the NDIS Practice Standards, Quality and Safeguarding Framework.
- Managing, supporting and providing feedback to Support Workers.
- Re-organising work priorities to meet the needs of a fast-paced environment.
- Identify and assist with new Participant referrals.
Essential Criteria:
- Relevant experience in a social service environment e.g. case management and advocacy.
- Ability to build a rapport and develop ongoing professional relationship with our participants.
- University or tertiary qualifications in social work, psychology, community services, disability services or nursing.
- Organisational, administrative and time management skills.
- Must share the values of the company using person-centred and strength-based approaches.
Highly regarded Criteria:
- Experience with assisting Participants through their NDIS journey and a thorough knowledge of the NDIS.
- Ability to apply relevant policies and procedures in the workplace, includes WHS procedures and implementing continuous quality improvement tactics.
- Sound understanding of the NDIS Practice Standards, Quality and Safeguarding Framework.
You will need to obtain the following prior to employment:
- National Police Check
- Blue Card
- Drivers Licence
- NDIS Worker Screening Check
- First Aid & CPR Certificate
- COVID-19 vaccination (desirable)
