Position: Caseworker (Full-Time)

Location: Newcastle Darby Street or East Maitland

Sunflower Services is a growing and dedicated registered NDIS provider serving the communities of Newcastle, Maitland, and the Hunter region. With a specialised focus on mental health and psychosocial disabilities, our organisation is committed to providing compassionate and effective support to our clients. Responsibilities for the Caseworker position include:

-Conducting intake assessments and developing comprehensive support care plans and risk assessments for new referrals.
-Listening to and assessing participants' needs, wants, and goals, with a focus on promoting their autonomy and well-being.
-Collaborating closely with the roster coordinator to develop suitable rosters for ongoing participant care, ensuring continuity and consistency of support.
-Overseeing one-on-one support sessions and actively seeking feedback from participants and support workers to continuously improve service delivery.
-Providing guidance, mentorship, and support to support workers, fostering a collaborative and cohesive team environment.
-Offering sound advice and assistance to participants and support workers during times of crisis or high-risk situations, while prioritizing their safety and well-being.
-Conducting debriefing sessions with support workers to reflect on challenging situations and identify areas for improvement.
Communicating effectively with relevant stakeholders, including mental health nurses, support coordinators, occupational therapists, and psychologists, to ensure coordinated and holistic support for participants.
-Working closely with the roster coordinator to manage staff sickness and leave, and offering appropriate replacements to maintain service continuity.
Writing advocacy letters for NDIS plan reviews and reassessments, advocating for participants' needs and rights.
-Undertaking various office administration tasks, including responding to emails and phone calls, reading and updating case notes, and preparing service agreements.
-Conducting in-home and community visits to assess participants' living conditions and support needs, and provide hands-on assistance and guidance as required.
-Assisting with business development activities, including marketing initiatives and participation in networking events, to promote Sunflower Services and attract new participants.
-Always approaching interactions with participants with a mindset of good intent and a focus on finding solutions to their challenges.
-Handling complaints, feedback, and concerns relating to support workers and community access supports promptly and effectively, ensuring a high standard of service delivery.
-Maintaining a strong focus on building positive relationships with stakeholders and support coordinators to generate NDIS leads and referrals and contribute to the growth and sustainability of Sunflower Services.
Demonstrating a comprehensive understanding of NDIS practices and updated legislation, and adhering to Sunflower's ethos and cultural values in all interactions and activities.
-Following the direction of the NDIS Service Manager and Operations Manager, and collaborating effectively with other team members to achieve organizational goals and objectives.


The ideal candidate for this role will possess the following qualifications and attributes:

A Diploma or Bachelor's degree in Social Science, Social Work, Human Services, or Psychological Science, or equivalent qualifications and experience (or working towards obtaining such qualifications).


Previous experience in support work or a related field will be highly regarded.
Excellent communication skills, both verbal and written, with the ability to convey information clearly and effectively to a diverse range of stakeholders.
Professionalism and integrity, with a genuine commitment to upholding ethical standards and maintaining confidentiality.


Strong literacy skills and attention to detail, with the ability to accurately record information and prepare written reports and documents.
Adaptability and resilience, with the ability to thrive in a dynamic and fast-paced work environment and effectively manage competing priorities.
Empathy and compassion, with a genuine desire to make a positive difference in the lives of individuals with disabilities and their families.
