The RoleWe are looking for a Part Time experienced Chronic Disease Management (CDM) Nurse to join our Fairfield practice to be the driving force and advocate for the local community in preventative health. This role will see you support GP’s and their patients to identify and manage their chronic conditions and ultimately keep them out of hospital. The TasksPrepare Care Plans and Team Care Arrangements to help patients manage their disease/sEmpower patients to increase ownership over their healthCoach patients on embedding health behaviour change principlesEnable patients to access ongoing care from a multi-disciplinary teamAssist in early identification of Chronic Disease by conducting health assessmentsWork with GPs to promote preventative healthcare throughout the practiceIncrease the number of patients who receive a care plan for the first timeEnsure patient’s care plans are reviewed regularlyManage administrative tasks relating to carrying out CDM appointment The Successful Candidate AHPRA registered Enrolled or Registered NurseProven prior experience in Chronic Disease Management and health assessmentsKnowledge of Health Assessment is highly regardedStrong organisational and time management skillsOutstanding communication and customer service skillsA passion for people and a personable approach The successful candidate will have the opportunity to be part of an outstanding work environment in a purpose and values-driven Company that invests in its people through training and development and internal promotion.
