Job description

We are currently seeking a passionate and motivated Client Assessor to join our Brightwater Regional Assessment Service team on a permanent full time basis.  The Regional Assessment Service (RAS) is a National service established by the Commonwealth Department of Social Services.

The RAS provides a standardised approach to in-home assessments for people aged over 65 or over 50 for Aboriginal and Torres Strait Islander people who require home support services to enable them to remain independent for as long as possible.

 

Key responsibilities:

  • In applying for this role, candidates must be able and willing to complete mandatory RAS training to achieve the National Statement of Attainment (SOA2) and have experience identifying client needs and conducting client assessments. 
  • Conduct online in-home assessments using the National Screening and Assessment Form
  • Complete assessments within a specified timeframe linked to KPIs
  • Develop the Support Plan in consultation with the client, carer and other relevant parties
  • Maintain a thorough knowledge of funded and non-funded Service Providers in the Region in which Assessments are undertaken 
  • Demonstrated ability of working respectfully with older people and their families 2
  • Demonstrated IT skills and competency in the use of mobile technology and online services
  • Ability to identify potential WHS risks within clients’ home environment 
  • Understanding of normal ageing processes and complex co-morbidities 
  • Ability to undertake a strengths-based assessment and facilitate goal focussed care plans 
  • Demonstrated ability to undertake culturally appropriate assessments 
  • Effective time-management ability 
  • Highly developed communication skills – verbal and written 
  • Commitment to promote and apply a wellness and reablement philosophy with older people 
  • Ensure regular review of the Gateway System Service Finder functionality to retain currency of knowledge of funded Service Providers 
  • Ensure a sound understanding of, and operate within the parameters of, the Regional Assessment Services Statement of Requirement and Guidelines, Contractor Standard Operating Procedures and internal policies and procedures 
  • Demonstrated ability to work under pressure and achieve set performance measures while maintaining quality standards
  • Most of the work will be conducted in the client’s home using online forms and databases. There will be a portion of office time for linkage activities, finalisation of documentation, reporting, supervision and team meetings. 

Desirable but not essential:

  • Existing knowledge and understanding of aged home care service providers
  • Previous experience in identifying client needs and conducting client assessments

You will also need:
