Position Overview: 

We are seeking skilled and experienced Client Acquisition and Intake officers for SDA Accommodation. You will be responsible for bringing in new participants, as well as building and maintaining positive relationships with participants, families, and support networks, ensuring their needs are met and preferences are understood and respected. 

You will oversee the client acquisition, engagement and intake process, assess individual requirements, and collaborate with internal teams to develop personalized support plans. This role requires strong interpersonal skills, strategic thinking, and a passion for enhancing the lives of individuals with disabilities.

Key Responsibilities:

Client Acquisition, Engagement, Intake and Assessment:

  • Coordinate the intake process for new participants, conducting initial assessments to understand their support needs, goals, and preferences.
  • Collaborate with participants, families, and support coordinators to gather relevant information and develop comprehensive support plans tailored to each individual's requirements.

Relationship Management:

  • Cultivate positive and trusting relationships with participants, families, and support networks, serving as a primary point of contact for inquiries, concerns, and feedback.
  • Provide ongoing support and guidance to participants and their families, advocating for their needs and preferences within the organization and the broader community.

Service Coordination and Planning:

  • Work closely with internal teams, including support workers, allied health professionals, and administrative staff, to ensure seamless service delivery and coordination of care.
  • Collaborate with participants and support teams to review and adjust support plans as needed, ensuring alignment with evolving needs and goals.

Quality Assurance and Compliance:

  • Monitor service delivery and participant outcomes to ensure compliance with organizational standards, regulatory requirements, and industry best practices.
  • Conduct regular reviews and evaluations of support plans, seeking feedback from participants and stakeholders to identify areas for improvement and innovation.

Community Engagement and Advocacy:

  • Represent the organization in community forums, stakeholder meetings, and industry events, advocating for the rights and needs of individuals with disabilities.
  • Promote awareness and understanding of SIL & SDA accommodation options, contributing to initiatives aimed at enhancing accessibility, inclusivity, and choice for clients.

Qualifications:

  • Certificate III and/or higher in Social Work, Disability Support, or related fields (or equivalent experience in new client acquisition role).
  • Previous strong experience in disability Support Coordination, Case Management, Social Work or Client Engagement roles, with knowledge of SIL & SDA accommodation models highly preferred.
  • Strong interpersonal and communication skills, with the ability to build rapport, resolve conflicts, and collaborate effectively with diverse stakeholders.
  • Excellent organizational and time management abilities, with a keen attention to detail and accuracy in documentation and record-keeping.
  • Commitment to upholding ethical standards, confidentiality, and person-centred approaches in service delivery.

Mandatory:

  • Australian work rights to be able to work in a full-time, permanent capacity
  • Australian Driver's license and a vehicle to be able to travel for Client Engagements
  • Working with Children Check, Relevant Disability Sector cards, Australian Federal Police check, NDIS Worker's screening, First-Aid Certificate
