Our approach is person-centred, and we are a "lifestyle-focused" care organization. We service all metropolitan Melbourne and have support workers located across all suburbs.

About the Role
- Accompany Manager for client visits
- Building client relations
- Building client profile
- Provide direct assistance to NDIS participants their families and understand the support they need
- Develop and maintaining a great network with referrers
- Create participants support plans that reflect the participant's needs and goal
- Undertaking client risk assessments to keep everyone safe
- Positively promote Care Mode Support Services as brand ambassador
- Provide prompt, courteous services to clients, external parties, colleagues, and the community
- Going above and beyond to strengthen our participants’ abilities to implement supports and participates fully in the community.
- Assist participants to work towards their goals in their NDIS plan
- Providing support and assistance as appropriate to relevant others/stakeholders
- Establishing and developing strong relationships with NDIS, community, and mainstream supports
- Provide Support Coordinators progress reports on clients when requested
- Work collaboratively with clients, coordinators, and colleagues.
Skills and experience
- Tertiary qualification in Mental Health, Nursing, Disability, Social Work, or relevant to the field of human services- Preferred not necessary
- NDIS awareness, particularly in relation to client services management and the importance of choice and control for participants
- Demonstrated ability to develop positive relationships with a range of participants, their families, and their service providers
- Well-developed communication (both oral and written) skills with the ability to prepare reports and correspondence in clear and concise language
- Strong interpersonal skills with an understanding of diversity when working with participants and colleagues
Appointments to Care Mode roles will be subject to the following safety screening checks:

- Current and valid driver's license
- Covid vaccination certificate
