As a Clinical Care Coordinator, you will be at the heart of our mission to provide exceptional care and support to our patients. Your role will encompass a range of responsibilities, including:- Collaborating with healthcare professionals to ensure the highest standard of patient care.
- Coordinating and managing patient care plans with empathy and attention to detail.
- Providing guidance and support to patients and their families throughout their healthcare journey.
- Monitoring and evaluating the effectiveness of care plans to drive continuous improvement.
