The Youth Clinical Care Coordinatorwill provide the wrap around services and supports often required by young people who are experiencing, or are at risk of, severe mental illness with complex care needs. The Coordinator is to provide ongoing support to young people - linking them to supports, assisting with key areas of need and connecting them to other services as required.
This role is Fixed term Until June 2025.
About the Role:
This is an exciting time for Headspace and a great opportunity for you to be a part of a growing, dynamic and supportive team of professionals providing mental health support for young people, 12 to 25 years.
This role is responsible for:
- To improve access to adequate level of primary mental health care intervention to maximise recovery and prevent escalation
- Provide wrap around coordinated care for young people with complex need
- Provide clinical mental health services to support needs of young people with severe and complex mental illness
- Provide service coordination, regular follow up and review of treatment plans
About You:
You will have demonstrated experience working with young people between 12 and 25 years of age as well as:
- tertiary qualifications in health, behavioral/social sciences (psychology, social work, or youth work
- skills in conducting risk assessments, develop action plans to mitigate risks
- experience in working in a multidisciplinary team
- knowledge of youth and mental health sector
