Clinical Care Coordinator

Clinical Care Coordinator | Full Time - Permanent | Mildura, VIC

We currently have a vacancy for 1 full time Clinical Care Coordinator to join the team at Regis Ontario. Reporting directly to the Clinical Manager, the role of the Clinical Care Coordinator is to provide and coordinate resident care and supervise care staff.

As a Senior Registered Nurse, you will be required to lead and manage the clinical and allied health services under the direction and support of the Clinical Manager. As a member of the clinical team you will perform in accordance with legislation and practice within a professional and ethical framework. The Clinical Care Coordinator will liaise with the Clinical Manager to ensure resident care outcomes are met.

Duties & Responsibilities

• Completing admission processes for residents
• Completing comprehensive assessment, care planning and continuing evaluation via Regis ROD program
• Management of complex health care procedures
• Consulting with residents, their families, medical practitioners and allied health professionals and coordinating care
• Completing audits of clinical processes, identifying and assist rectify deficits
• Providing education and coaching to Floor Clinicians

Skills & Experience

• Strong verbal and written communication skills
• AHPRA Registration and a minimum of 12 months experience as a Registered Nurse in Australia
• Previous experience working in Aged Care (preferred)
