About the Role: As a Clinical Care Coordinator, you will play a pivotal role in providing clinical leadership and ensuring the delivery of top-notch care services. Working closely with the Facility Manager and other key stakeholders, you'll oversee and guide the implementation of resident-focused clinical strategies to achieve positive outcomes.
Key Responsibilities:
- Utilize your robust clinical skills to maintain high standards of care delivery.
- Provide leadership, mentorship, and supervision to our clinical team.
- Conduct comprehensive assessments and develop tailored care plans for residents.
- Ensure compliance with AN-ACC funding model and Aged Care Quality Standards.
- Current registration with the Australian Health Practitioners Agency (AHPRA).
- Proven experience as a Registered Nurse in Aged Care with strong clinical acumen.
- Previous experience in a residential care setting is essential.
- Genuine passion for enriching the lives of elderly individuals.
- Familiarity with AN-ACC funding model and Aged Care Quality Standards is desirable.
